Hydrosalpinx is a condition that causes fluid to collect where the fallopian tube meets with the ovary. When this happens, a blockage of the fallopian tube is created and infertility becomes a major factor.
Hydrosalpinx can be brought on by many different things. Most of the time, the cause is PID, or Pelvic Inflammatory disease, but this is not the only thing that can cause a hydrosalpinx. It can also be brought on by scar tissue left behind from a previous surgery, abortion, infection, the use of intrauterine devices, and even childbirth itself can lead to that collection of fluid.
Another cause is when sexually transmitted diseases or infections are not detected early. Infections such as chlamydia and gonorrhea can often cause damage to the fallopian tube over a significant amount of time. When this happens, patients most often complain of severe pain in their lower abdomen and the pelvic region.
Hydrosalpinx can be detected by ultrasound,performance of a hysterosalpingogram (HSG),laparoscopy or by laparotomy.Ultrasound and HSG are preferred more since they are minimal invasive processes.Laparoscopy and laparotomy are surgical processes hence there is a risk factor in it.
It is important to detect hydrosalpinx at a nascent stage itself as it can cause infertility.The fallopian tube may become useless and the movement of sperm or egg from ovary through the fallopian tube is at risk.The fluid which exudes from the fallopian tube is toxic in nature for the embryo and therefore any treatment for infertility becomes ineffective.The chances of miscarriage also increases.
Infertility treatments such as IVF can be 50% less effective when a hydrosalpinx is present. Yale University conducted a study that compared the results of thousands of IVF cycles. Their findings showed that women with a hydrosalpinx were twice as likely to miscarriage and successful implantation was 50% less likely to occur.
The condition of having hydrosalpinx can be treated either surgically or non-surgically.The surgical treatment involves making an incision in the tube,commonly referred as salpingostomy or complete removal of tube,which is known as salpingectomy.In such cases,there are chances of incision getting closed,and it can cause a recurrence of hydrosalpinx.
Salpingostomy is more effective when the hydrosalpinx is small and when it is detected at an earlier stage.Salpingectomy is a prophylactic surgery and results have shown substantial improvement during pregnancy and birth rates.
Removal of the affected fallopian tube by the intervention of surgery is recommended when the patient is trying to conceive through in-vitro fertilization.However,taking into account the risks associated with surgery and the psychological aspects of having a tube severed,it is necessary that the patient goes through proper counselling sessions and consents prior to the operation getting performed.
The preferable non-surgical treatment does not required the recommendations of surgical treatment. In these situations, pelvic physical therapy may assist in removal of blockages. The therapy is a massaging of the tubes so that the blockage can open. The success rate increases by 70% with this treatment when used with fertility yoga, natural supplements and herbal tampons.
